ACCOUNTING

FAX AUTHORIZATION

Fax Authorization Form
Due to the new Federal Communications Commission (FCC) regulation that created the National Do-Not-Call Registry we are required to have written authorization on file in order for us to fax information to you our clients, prospects, and vendors.  Because of it's very broad definition of an "unsolicited advertisement", we require all our clients, prospects, and vendors to execute this form and fax it to us before we can communicate directly with you.

INDEPENDENT CONTRACTOR'S

IRS Form W-9, Request for Taxpayer Identification Number and Certification
This form is used to obtain the Federal Tax Identification Numbers of your vendors.  This form should be completed by all your vendors whether or not you are required to send the a form 1099.  This form can be filled in on your screen and printed.  Make sure you complete the Requester's Name box so that we know which client this completed form belongs to.  W-9 Instructions

Cover Letter for IRS Form W-9, Request for Taxpayer Identification Number and Certification
We have provided a cover letter you can place on your company letterhead and change appropriately for sending your IRS form W-9 to your vendors.

OFFERS IN COMPROMISE

IRS Form 433-A, Collection Information Statement for Individuals
This form must be completed for any individual, joint, or self-employed taxpayer desiring to submit an Offer In Compromise to the IRS.  This form can be filled in on your screen and printed.  Please sign the form and compile all supporting information as requested on the form prior to sending this to our office.

IRS Form 433-B, Collection Information Statement for Business
This form must be completed for any Business desiring to submit an Offer In Compromise to the IRS.  This form can be filled in on your screen and printed.  Please sign the form and compile all supporting information as requested on the form prior to sending this to our office.

PAYROLL

IRS Form W-4, Employee's Withholding Allowance Certificate (2007)
This form is required to be completed by each employee and tell the employer what filing status and number of exemptions to use in calculating payroll tax withholdings.

IRS Circular E, Employer's Tax Guide (2007)
This document is prepared by the IRS and has the current Wage Withholding and Advance Earned Income Credit Payment Tables.


U.S. Department of Justice, Immigration and Naturalization Service, Form I-9, Employment Eligibility Verification
This form must be completed for each employee.  Failure to obtain this form could result in significant penalties being assessed against your business.
